Instructions
To complete our intake and authorization forms, please follow the below instructions. If you have any questions, please call our office.
- Download the PDF to your computer
- Open the downloaded file using your installed PDF reader
- Fill out all information
- Note: authorization forms will require an electronic signature, you can find this option within your PDF reader.
- Save the document to somewhere safe on your computer
- Log-in to our Client Portal and upload the completed form to the “Client Documents” folder
If you do not already have a Client Portal please e-mail admin@sternatcpa.ca for a registration link.
You can also print the form and complete by hand. If you chose to do so please provide the filled form with any documents you are dropping off at our office.
Please do not e-mail your form as it is not secure.
